This visit is intended after a Lifestyle Medicine Blueprint or 12-Week Lifestyle Change Program. It does not include a new comprehensive intake or initial assessment.
Lifestyle medicine complements appropriate medical treatment; it does not replace primary care, urgent care, or specialist care. One identified patient per visit. Virtual follow-up care in Massachusetts and Florida, available in English and Spanish.
