A Personal Journey That Hit Home
I was a rheumatology fellow seeing patients with my mentor during my Monday afternoon clinic when my father called.
"They found a mass in my chest," he said.
He was at the Mayo Clinic for his yearly check-up. Given his history of mantle radiation for Hodgkin's lymphoma as a teenager, I immediately feared a secondary cancer. I took a deep breath, closed my eyes, and my medical training took over. The textbook causes—the "four T's"—flashed before me: "teratoma, thymoma, thyroid mass, and terrible lymphoma".
Initial tests suggested metastatic kidney cancer. His doctors were preparing to start chemotherapy, but I wanted a second opinion.
The Power of a Second Look
I arranged for him to see a kidney cancer specialist in Boston. The doctor was thorough and honest, but before we left, he said something that changed everything: "Oh, and I would recommend another biopsy". He explained that when he reviewed the first biopsy sample with a pathologist, it contained very few cells, so it wasn't entirely clear the tumor had originated in the kidney.
Weeks later, the new biopsy revealed the truth: it wasn't kidney cancer but a very rare thymic carcinoma—one of the T's I had feared from the start.
This correct diagnosis was critical. Unlike with the initial diagnosis, surgical resection for thymic carcinoma could significantly improve his outcome. He even qualified for a clinical trial of immunotherapy, which gave him at least a year of tumor stability before other treatments followed. I often wonder what might have happened if we had accepted the first diagnosis without question.
